Job Description :

Join an interdisciplinary team of caring individuals to work with patients in their homes. Social Worker (SW) needed full-time to provide information on advance directives, referrals for case management, assistance with long term care planning, access to community resources, crisis intervention and short term counseling related to adjustment management of illness. The SW serves as a liaison between patient/clients under case management with other community services and provides services in accordance with the orders of the attending physician and the policies of VNA. SW provides information to terminally ill patient/clients on advance directives, referrals for case management, assistance with end of life issues, access to community resources, crisis intervention and grief counseling, provides support to families/caregivers of Hospice patient/clients, and is an active participant of the inter-disciplinary group.

Actual pay rate determined based on experience.

QUALIFICATIONS: Masters in Social Work (MSW) or Baccalaureate degree in Social Work, Psychology, Sociology or other field related to social work with one year’s experience. Must be in good standing with discipline specific licensing/certification agency and able to meet all federal and state licensure requirements.

Visiting Nurses requires a criminal background check and motor vehicle (MVR) check for all employees once a conditional offer of employment has been extended.
