Do you enjoy helping others and have a background working with a vulnerable population?? TriSource has the job for you! TriSource is working with a fantastic organization in the Anne Arundel county area seeking Case Managers!
Must have experience working with the DDA population or providing case management services.
Responsibilities:
- Develop and implement tailored Individual Support Plans (ISP) and ISP meetings
- Research for job/volunteer opportunities and enriching community-based activities for the people supported
- Communicate with all necessary parties; internal and external
- Conduct routine check-ins with caregivers
- Arrange/provide transportation as needed
Qualifications:
- Bachelor’s degree OR 4yrs relevant experience
- Ability to pass background check and drug test
- Have a driver’s license and personal vehicle
