Job Description :

Case Management

Essential Duties & Responsibilities

· Conduct home visits and follow up telephone calls for our clients

· Complete assessments and create care plans for in home services

· Assist the clients to find the services and providers to meet their needs

Qualifications:

· Baccalaureate in Social Services, Human Services, or a Registered Nurse with at least 2+ years’ experience in related field

Position Requirements:

· Valid Driver’s license

· Successfully pass a background check

· Computer experience

Job Type: Full-time

Pay: $37,000.00 – $40,000.00 per year
